Reported by peth. At the moment it's not working for custom mappings though!
Build and maintain a mapping table for extended mode the same way it's
already done for command mode ("normal mode").

I've added :cmap and :cunmap commands to vim-mode. They support ex-mode and irssi mode mappings, normal mode mappings are not (yet) supported.
